Six pillars. One accountable partner.
Every engagement is built from these six areas. Most providers start with two or three and add the rest as capacity and confidence grow.
Census growth & professional outreach
We do the outreach your team often does not have time to do.
With your written authorization, we introduce your services to the professionals involved in referrals, care transitions, and provider selection, communicate your current capacity and admission criteria accurately, follow up, and keep those relationships warm over time. Every contact is logged and reported.
What this includes
- Referral-source mapping and prioritization
- County and Tribal outreach
- Case manager and care coordinator outreach
- MnCHOICES assessor outreach
- Hospital and discharge-team outreach
- Hospice, home health, and clinic outreach
- Authorized outreach calls
- Professional email campaigns
- Structured follow-up
- Capacity and vacancy updates
- Ongoing relationship management
- Presentations and in-services
- Referral-source database management
- Outreach tracking and activity reporting
Digital marketing, SEO & PPC
Be the provider they find, and the one they believe.
Families and professionals search before they call. We make sure what they find is accurate, current, and persuasive — so families and professionals searching in your county can find you more easily.
Advertising budgets are separate from our management fees and are paid by you directly to the platform.
What this includes
- Website creation or improvement
- Local SEO for your counties
- Technical and on-page SEO
- Google Business Profile optimization
- Google advertising and PPC management
- Appropriate social advertising
- Landing pages for campaigns
- Content strategy
- Social media management
- Local search visibility and listings
- Analytics and performance reporting
Reputation & reviews
A discharge planner checks your reviews before they call you.
Reputation belongs inside census growth, not beside it. We build the systems that generate honest reviews, catch problems early, and make sure every review gets a professional response.
This is also the pillar that keeps earning when you are full. A strong review history can improve trust and visibility when the next opportunity arises.
What this includes
- Reputation monitoring across platforms
- Review-generation strategy and systems
- Google reputation management
- Review-response support
- Profile optimization
- Customer-experience feedback systems
- Reputation reporting
Brand & creative
Something good to leave behind in a county office.
We design and produce the materials your team hands out, in your brand. If you already have an identity we work within it and strengthen it. Source files and print-ready artwork are delivered to you and remain yours.
What this includes
- Brand development
- Logo and identity support
- Brochures and flyers
- Banners and signage
- Business cards
- Referral packets
- Capacity and admission one-pagers
- Professional presentations
- Digital and social graphics
- Facility marketing materials
Inquiry & CRM management
Fast, organized response matters.
We set up the line, answer the inquiry, log it, route it to your admissions decision-maker, and follow up. We may establish and configure a dedicated number on your behalf, but the number and the account are yours: you retain access, we manage it during the engagement within the authorized scope, and it stays with you afterwards. We identify ourselves as working on your behalf, never as the care provider itself.
What this includes
- Dedicated outreach phone setup
- Provider-controlled phone and email systems
- Inquiry tracking and organization
- CRM setup and management
- Follow-up workflows
- Response-time tracking
- Referral-source attribution
- Pipeline visibility
- Past inquiry re-engagement, where appropriate and permitted
- Reporting as defined in your proposal
Dedicated outreach phone numbers and communication accounts can be established for the provider and remain provider-controlled. Any re-engagement of past inquiries follows applicable consent, privacy, communication, and provider requirements.
Provider operations support
The part most marketing agencies cannot help with.
Non-clinical administrative support that keeps the business organized: documents where they should be, renewal dates tracked before they lapse, credentials and training logged, workflows written down.
This is where the relationship goes once occupancy improves. Reputation, search visibility, recruiting, dashboards, and administrative systems keep producing value long after the beds are full — and they are what keep them full.
Scope note. We provide administrative organization and readiness support. We do not guarantee regulatory compliance, and we do not replace your own legal, clinical, licensing, or regulatory responsibilities or the professionals who advise you on them.
What this includes
- Document and folder organization
- Policy and document library
- Renewal and license-date tracking
- Staff credential and training tracking
- Document expiration tracking
- Safety-documentation organization
- Administrative checklists
- Readiness organization
- Business process improvement
- Operational dashboards
- Workflow development
- Management consulting
- Staff recruiting marketing
- Ongoing SEO and digital presence
- Ongoing reputation management
- Business consulting and growth planning
Provider-owned tools and third-party costs
When the work requires a dedicated phone number, domain, website hosting, CRM, advertising account, email platform, printing service, software subscription, or another third-party service, we establish it in your organization's name whenever practical. You retain ownership and control of provider-specific accounts and assets. Census Partner may configure or purchase approved services on your behalf, but third-party costs are separate from our service fee unless your written proposal specifically states otherwise.
Third-party expenses may include Google Ads spend, social advertising spend, phone service, SMS and usage charges, CRM and software subscriptions, domain registration, website hosting, email platforms, printing, photography or video, outsourced production, premium integrations, and other provider-approved services. Third-party expenses are approved by the provider and billed separately unless expressly included in the written proposal.
Not sure which pillars you need?
That is what the consultation is for. We will tell you where the gap actually is, even if it is somewhere we do not sell.
