We do not assess individuals, determine eligibility, approve services, or make placement decisions. Every admission decision is made by the care provider, and every placement decision is made by the individual and their care team.
We do not guarantee that any provider will have availability, will accept an individual, or will be an appropriate match. Provider information, including availability, changes frequently and may not be current.
We work with a limited number of providers, and other qualified providers exist. Verify license status and survey history independently — Minnesota publishes this free through the MDH provider directory, the DHS Licensing Information Lookup, and the Assisted Living Report Card.
Care providers pay us a fixed monthly fee. Our compensation does not vary based on whether you contact a provider, choose one, or move in. See our Trust Center.
Provide accurate information; do not submit another person's information without their permission or your legal authority; do not submit medical records or detailed health information; do not use the site unlawfully or to harm others.
