Who we are

Census Partner is a division of Resultez Consulting Group LLC, a Minnesota limited liability company. We provide marketing, outreach, and business support services to licensed residential care providers in Minnesota. We are a private company — not a government agency, a county, a health plan, or a case management service.

What we collect

• What you give us: name, phone, email, ZIP or county, who the inquiry is for, the general type of care sought, approximate timeline, and how care may be paid for.
• Collected automatically: IP address, browser type, pages visited, referring site.
• We do not ask for and do not want diagnoses, medication lists, medical records, Medicaid identifiers, or other detailed health information.

How we use and share it

• To respond to your inquiry, identify providers that may be an appropriate match, share your inquiry with providers you authorize, schedule consultations or tours, and improve our services.
• With care providers you authorize — we tell you which providers before we share.
• With service providers who help us operate (hosting, communications, scheduling) under written contracts limiting their use.
• When required by law, or to protect safety.

We do not sell your personal information. We do not share it with providers you have not authorized. We do not send it into consumer AI tools, and we do not use it to train any AI model.

How we are paid

The care providers we work with pay us a fixed monthly fee. Our fee does not change based on whether you choose a provider, whether you move in, or how long you stay. We receive no placement fees, commissions, or per-resident payments. We do not charge families.

Your choices

• Ask us to correct or delete it
• Withdraw permission for us to contact you
• Withdraw permission to share with a provider

Retention, security, children, changes

We keep inquiry records for [__] years and consent records for five years as federal rules require, then delete on schedule. We use encryption in transit and at rest, multi-factor authentication, role-based access limits, and audit logging. This website is not directed to children under 13. Changes are posted here with an updated effective date.
