Who we are

A Minnesota consulting firm. Not a provider, not a county, not a placement agency.

Census Partner helps licensed assisted living facilities and group homes in Minnesota fill and keep their beds full — through marketing, professional outreach, and fast, well-organized intake. We are a private company. We are not a government agency, a county, a health plan, a case management service, or a licensed provider. We do not make clinical, eligibility, or admission decisions.

How we're paid

A flat monthly fee. Never per resident, per bed, per admission, or a percentage.

Our fee is the same whether we help you fill zero beds or ten. It is set in writing before we start and does not change during your term based on results.

This is not a marketing preference. The federal Anti-Kickback Statute (42 U.S.C. § 1320a-7b) and Minn. Stat. § 609.542 make it a felony to pay or receive anything of value in exchange for referring individuals whose care is paid by Medicaid, including Minnesota waiver programs. Our agreements are written to the federal safe harbor at 42 C.F.R. § 1001.952(d)(1) — fixed fee, set in advance, at fair market value, written scope, term of not less than one year. We complete and sign a written fair market value determination before every contract.

You can hand our contract to a DHS reviewer without flinching. That is the point.

Commitments

What we will never do.

Published verbatim, and repeated in every agreement we sign.

Never charge you per lead, per inquiry, per tour, per admission, per resident, or per bed filled.

Never take a percentage of your revenue, your rent, or your reimbursement.

Never give anything of value to a case manager, MnCHOICES assessor, MCO care coordinator, discharge planner, social worker, guardian, or public employee. No cash, no gift cards, no meals beyond a modest working lunch at a genuine educational session, no free software.

Never accept payment from a referral source.

Never accept a resident list, census file, or discharge list for marketing.

Never steer a family, pressure a referral source, or limit anyone's choice of provider.

Never hold your accounts, domain, or data hostage — including in a billing dispute.

Never claim to be a clinician, case manager, county, or government agency.

Privacy, security, and AI

We operate a default no-PHI model.

We don't need identifiable health information to do our work, so we don't take it. If a referral source begins sharing client details, we stop them and direct the inquiry to your intake contact. Consumer inquiries and provider information are kept in separate systems with separate credentials, and we run no advertising trackers on any page where someone tells us about their care needs.

We use AI to answer the phone around the clock, respond to inquiries in under five minutes, draft follow-ups, score inquiries for fit, and prepare reports. Our automated assistants always identify themselves as automated, and a person is always available. AI never makes a clinical, eligibility, or placement decision. AI never ranks providers by who pays us. Your data is never used to train any AI model, and that is in writing with every vendor.

If our work ever requires handling protected health information on your behalf, we sign a Business Associate Agreement first. We will not accept it without one.

We screen every provider we work with against the OIG exclusion list, SAM.gov, and the Minnesota DHS excluded provider list before signing and monthly thereafter.
