We maintain current capacity and stated admission criteria for the licensed Minnesota providers we work with. Give us the county, service type, payer, and general support needs. We review the current capacity and stated admission criteria of participating providers and identify providers whose criteria may align with the request, usually within one business day.

No names, dates of birth, diagnoses, medication lists, Medicaid or MA identifiers, service plans, medical records, or other unnecessary identifying information. General service needs only. Once we identify providers whose stated criteria may align, we connect you directly with that provider's intake contact and you share details with them through your own secure channel. The provider makes its own admission decision, and provider choice remains with the individual, their authorized representative, and their care team.
How we are paid, so you know
Providers pay Census Partner a fixed monthly fee. That fee does not change based on whether you contact a provider, whether anyone is admitted, how long they stay, or how care is paid for. We receive no placement fee, commission, or per-resident payment, and we never give anything of value to a referral source.
