Who we are
Census Partner is an outsourced growth and provider-support partner for licensed assisted living facilities and group homes in Minnesota. The brand is Census Partner; agreements are held by Resultez Consulting Group LLC, a Minnesota limited liability company and a Minnesota-certified diverse, minority-owned business.
We are a private company. We are not a government agency, a county, a health plan, a case management service, or a licensed care provider. We do not make clinical, eligibility, or admission decisions.
How we're paid
Our fee is agreed before we start, set in writing, and does not change during your term based on results. We do not accept placement fees, commissions, finder's fees, or per-resident payments — from anyone. Families and individuals never pay us anything.
We complete and sign a written fair market value determination before every contract.
Our commitments
- Never charge per lead, per inquiry, per tour, per admission, per resident, or per bed filled.
- Never take a percentage of your revenue, your rent, or your reimbursement.
- Never give anything of value to a case manager, assessor, care coordinator, discharge planner, social worker, guardian, or public employee in connection with referrals.
- Never accept payment from a referral source.
- Never accept a resident list, census file, or discharge list for marketing.
- Never steer a family, pressure a referral source, or limit anyone's choice of provider.
- Never hold your accounts, domain, or data hostage — including in a billing dispute.
- Never claim to be a clinician, case manager, county, or government agency.
Privacy, security & AI
We do not need identifiable health information to do our work, so we do not take it. We do not request health information through our public forms, and if a referral source begins sharing client details, we stop them and direct the inquiry to your intake contact.
We use automation to respond quickly, organize follow-up, and prepare reports. Our automated assistants always identify themselves as automated, and a person is always available. Automation never makes a clinical, eligibility, placement, or admission decision, and never ranks providers by who pays us. If our work ever requires handling protected health information on your behalf, we sign a Business Associate Agreement first.

We screen every provider we work with against the OIG exclusion list, SAM.gov, and the Minnesota DHS excluded provider list before signing, and monthly thereafter.
